Home School Field Day

This Release of Liability is to cover Field Day 2026. Participants may attend only when they are free from illness. I understand that free from illness includes being free of fever or cough and also includes the ever evolving list of additional symptoms listed by the CDC. I understand that this release discharges Carter County Drug Prevention from any liability or claim, and holds Carter County Drug Prevention harmless, that I, my heirs, or any personal representatives may have against Carter County Drug Prevention with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any participation in activities or events. This liability waiver and release extends to Carter County Drug Prevention together with any others associated with Carter County Drug Prevention including Elizabethton Parks and Recreation.

I hereby grant permission to Carter County Drug Prevention and/or any affiliated organizations to use my: Photograph, Videotaped Image, Quotes/Comments, Name, etc. for publicity and educational purposes in any and all publications and media without limit or reservation.
