Flag Football Toy Drive

I, the undersigned, hereby acknowledge that my participation in the Flag Football Game on November 22nd at Unaka High School involves a risk of physical injury. In consideration of being allowed to participate in this event, I agree to the following:

Assumption of Risk: I understand that flag football is a physical sport and I voluntarily assume all risks associated with my participation, including but not limited to falls, contact with other players, and field conditions.

Release of Liability: I hereby release, waive, and discharge Carter County Drug Prevention, Unaka High School, and all associated event organizers, volunteers, and sponsors from any and all claims, liabilities, demands, or causes of action arising out of my participation in this event, including any injury or damage to personal property.

Medical Consent: I grant permission for emergency medical treatment to be administered if I am injured during the event and unable to consent at the time.

I have read this waiver, fully understand its terms, and sign it voluntarily.
